11.  Payment for services rendered.  You agree that you are responsible for the prompt payment of any service or product, including, but not limited to, eye examination, treatments, eye wear, contact lenses, optician services, repairs, adjustments, and any other services or products provided by Uncommon Eyewear LLC.  You understand that Uncommon Eyewear LLC does not accept or bill insurance, including Medicare or Medicaid, and that all payment is due at the time the service is rendered.  In some cases, Uncommon Eyewear LLC will require payment or a deposit at the time of scheduling or booking a service.  Failure to pay for services is considered to be theft and will be referred to local law enforcement and will be subjected to civil collection and adjudication procedures as allowed by law.  

12.  Complete agreement.  These Terms and Conditions, together with any Additional Terms and Conditions incorporated by reference, constitutes the entire agreement between you and Uncommon Eyewear LLC related to its website and products and services.  The failure of Uncommon Eyewear LLC to exercise or enforce any right or provision in these Terms and Conditions shall not constitute a waiver of such right or provision.
